Record of Incomplete Grade

This form is to be completed by the course instructor.  Do not include any medical/health related information or the student's current grade, in this form.  If necessary, this type of information can be sent by email to chemistry@uky.edu from an instructor's UKY email account.

The instructor, student, and department will recieve copies of the submission.  There is an option to add more email recipients as needed. Only use UKY domain email addresses (i.e. addresses ending in uky.edu).  Please be aware, no information is stored on servers.  The emails constitue record of the contract.
